Lost my pacemaker card: how to get a replacement
Contact the clinic that checks your pacemaker and ask for a replacement identification card. If you do not know which clinic to call, start with the hospital where it was fitted or ask your GP to help locate the relevant team. You do not need to buy a digital passport to request your device details.
How to request a replacement pacemaker card
- Find your device clinic. Look on an appointment letter or hospital correspondence. Use the hospital’s official website or switchboard if you need a current number.
- Explain what is missing. Say that you need a replacement card and a readable copy of your current implant details. Mention any upcoming scan, treatment or journey.
- Ask how they will send it. Check the address or secure route they will use and what you should do while waiting.
The British Heart Foundation advises contacting your pacemaker clinic if you do not have a card. Some manufacturers also offer replacement services; the process depends on the manufacturer and country. Start with your UK clinic rather than submitting personal details to an unfamiliar overseas form.
A message you can use when contacting your clinic
I have lost my pacemaker identification card. Please could you arrange a replacement and confirm my current device and any lead details? I have an appointment or journey coming up on [date]. Please let me know how I can receive the information securely.
Use your clinic’s usual secure contact route. They may ask for identifying details to find your record. Avoid posting your card, date of birth or hospital number in a public forum.
If you have an appointment before the card arrives
Tell the team arranging your appointment that you have a pacemaker and are waiting for its details. Give them your device clinic’s contact information. An old card or photo may help them trace the record, but explain if the device has since been replaced.
Do not guess a model from an internet image. If an MRI is being arranged, read what the MRI team needs to check. Never delay emergency care to find a card.
Once you have your details, keep a backup you can find
Keep the replacement clinic or manufacturer card. A clear photo is a useful digital backup. If you want the information organised into readable fields, ImplantPassport lets you turn a card into a digital record, check the entries and keep a copy ready for appointments.
Our optional NFC card is an ImplantPassport access card. It is not an official replacement card issued by your pacemaker manufacturer or hospital.
Questions you may have
Can ImplantPassport recover my missing pacemaker details?
No. The card reader works from information you provide; it cannot retrieve an unknown implant from NHS or manufacturer records. Ask your clinic for the source details first.
Can I use an old card after a pacemaker replacement?
Keep it as part of your history, but ask your clinic to confirm the current record. Clearly identify an old card as historical when showing it to another team.
